Analytic Method

We propose to use our existing arrays for biomarker evaluation but to focus the evaluation on 9 candidates: KLK5,CRHBP, TFRC, ITGB4, RASGRF2, MUC5AC, FOLR3, YWHAB, CNOT7. These include the 8 marker panel described above that achieved 35% sensitivity at 5% specificity and the one additional marker from our A list that validated in our independent preclinical OS-WHI samples, CRHBP. The primary data analysis will focus on the combination of these 9 markers derived by logistic regression from the OS-WHI samples.

Outcome

A plasma protein marker score based on a linear combination of ten plasma protein measures developed previously for discrimination of ER+ breast cancer (BCa) from benign breast disease (BBD) was evaluated for validation with EDRN Breast Reference set specimens. There was no evidence for discrimination by this score between subjects with invasive ER+ BCa and BBD in this sample, with estimates of sensitivity at 80% specificity = 21.4% (P=.81), AUC = .504 (p = .91), and LRT p = .86. There was an additional exploratory evaluation of 120 protein markers, 461 autoantibody features, and 184 glycomic markers using the same samples. Plasma specimens were from 271 subjects in the EDRN Breast Reference Set including 112 ER+ invasive breast cancer cases and 159 non-atypical benign breast disease controls. ROC statistics (AUC & sensitivity @ spec = .80) used pv-based calculations and were stratified on hybridization day. LRT statistics are based on logit models which included a factored covariate for hybridization day.
